Instead of collecting power ups or coins, you grab insulin shots and healthy snacks like apples to keep your energy stable. If you eat the wrong thing or miss your insulin, your health plummets and the character collapses. The pacing is deliberate, almost tense, because a single mistake can undo your progress. It feels like a strange, earnest attempt to turn a serious health condition into a playable mechanic.
