Life and Death 2: The Brain came out in 1990 as part of the MS-DOS Classic Games series, published by The Software Toolworks. It was one of those early simulation titles that tried to capture a realistic, high-stakes profession on the computer, back when that kind of thing still felt novel and a little bit daring. You don't see many games like it anymore, where the goal is to perform actual surgery instead of shooting aliens or jumping on platforms.

You play as a neurosurgeon, controlling a cursor to select tools and perform delicate operations on a patient's exposed brain. The main objective is to successfully diagnose and remove tumors or blood clots without causing fatal damage. Key mechanics include navigating a detailed anatomical map of the brain, using specific surgical instruments like scalpels and suction devices, and constantly monitoring the patient's vital signs. The pacing is tense and deliberate; every action has immediate consequences, and mistakes are punishing. It feels like walking a very fine line between success and disaster with nothing but your own steady hand to rely on.

Each game uses different controls, most DOS games use the keyboard arrows. Some will use the mouse , "Alt" ,"Enter" and "Space bar".
