Life & Death 2 – The Brain came out in 1990 as part of the MS-DOS Classic Games series, published by Software Toolworks. It was one of those early attempts to blend education with entertainment, putting you in the role of a neurosurgeon at a time when medical simulations were still a novelty on home computers.

You control the surgeon’s tools, performing procedures like tumor removals and hematoma evacuations on a static, screen-by-screen patient view. The main objective is to diagnose and operate successfully without killing the patient; you use a cursor to select instruments, make incisions, and respond to changes in the patient’s vital signs. Signature mechanics include interpreting pre-op brain scans to locate issues and managing real-time physiological feedback during surgery. The pacing is tense, and the difficulty is high, with little room for error. It feels like walking a tightrope where one wrong move has immediate, irreversible consequences.


Each game uses different controls, most DOS games use the keyboard arrows. Some will use the mouse , "Alt" ,"Enter" and "Space bar".
